The success of operative treatment of gastric and duodenal ulcer bleeding depends upon the control of bleeding, relief of pain, re-establishment of the gastric barrier function and prevention of ulcer recurrence. This study was designed to evaluate the efficacy of vagotomy and pyloroplasty in the management of peptic ulcer bleeding. The data of 100 consecutive patients, admitted to the Endoscopic Unit of the University Hospital of Parma, Italy, for surgical treatment of active peptic ulcer bleeding were analysed. For 60 patients the bleeding was associated with endoscopic varices, and for 40 patients the bleeding was due to anticoagulant therapy. In the 20 patients with varices, bleeding was controlled by first intention suture (n = 7), injection of sclerosing agent (n = 7) and suture with simultaneous varices variceal banding (n = 4). The 20 patients with anticoagulant therapy were treated with a modified pyloroplasty and vagotomy. The operation was considered to be successful if the patient was pain-free after 48 hours and if endoscopy confirmed the disappearance of the varices.
